Advanced bone healing protocols in immune dentistry

Dr Miguel Stanley has been performing guided bone regeneration and tissue engineering always adopting the latest scientifically proven innovations at the White Clinic in Portugal, where he practices exclusively. Well known for only using the best technologies and materials, Dr Stanley and his team recently…

…have been developing specific pre-and post op protocols that enhance tissue regeneration and improve healing allowing better overall results.

Do we need to rethink the way we prepare the bone for implant placement? Are we properly removing medullary infection and inflammation prior to grafting? What technologies and techniques can we apply to improve outcomes?

There is some mounting evidence that osteoimmunology is a field that needs to have a bigger role in the toolkit of the modern dental surgeon. We need to understand the immune system and how inflammation interacts with the body. We have a unique opportunity to help our patient heal better at the time we place implants or regenerate the bone, the way we do it and the materials we use, can dramatically influence outcomes.

Guided bone regeneration (GBR) – correlating the appropriate choice of biomaterial with specific clinical scenarios

Guided bone regeneration (GBR) was developed in the late 80’ based on the guided tissue regeneration. Quickly it become the most popular technique for bone regeneration due to its versatility: the same technique can be used to treat small periimplant defects like dehiscence or fenestrations, or large…

…horizontal defects where simultaneous implant placement is not possible and a staged approach is mandatory.

The popularity of the GBR technique lead to the development of different biomaterials and membranes. Today, the GBR technique is the mostly documented technique in the literature. Still, there is a lot of confusion and uncertainty among practitioners regarding the choice of biomaterial suitable for different clinical scenarios.

Surgical techniques for reconstruction of severe ridge atrophy of the partial edentulous posterior mandible

The partial edentulous posterior mandible is a challenging area that often requires bone reconstructive surgery for implants placement. Alveolar atrophy is a pathological condition characterized by moderate or severe resorption of alveolar bone due to teeth loss. The loss of teeth determines the loss of the…

…functional stimulus for the alveolar bone.

Consequently, the bone undergoes constant and predictable resorption, which differs depending on location: it is vertical and outward in the posterior mandible. Alveolar bone resorption and the inferior alveolar nerve (IAN) location make the posterior regions of the mandible the most challenging to treat while using osseointegrated implants.

This lecture focuses on surgical techniques, natural tissues, and biomaterials for vertical and or horizontal bone augmentation to successfully restore edentulous ridges with implant-supported prostheses.

Bone-augmentation techniques proposed to increase bone volume in the posterior regions of the mandible will be presented, including implantations lateral to the IAN canal, transposition of the inferior alveolar nerve (IAN) with augmentation and simultaneous implants placement, guided bone regeneration (GBR) using titanium mash and onlay autologous block bone grafts (AOBG) for horizontal and vertical augmentation.

I-med full arch concept: all-on evolution

The I-med full arch concept is a graftless treatment protocol for severely resorbed maxilla. Using special surgical techniques and special implants, we can treat even extremely resorbed upper jaws without grafting and with immediate function. Combining standard implants, tilted long implants (All-On-4), zygomatic implants…

(quad, single or unilateral), pterygoid implants and short subnasal implants, we can achieve a  solid foundation for an implant supported screw retained immediate full arch bridge.

The foundation for this treatment is our standard I-med all-on protocol – a one stage surgery under general anaesthesia, extractions, implant and abutment insertions and an impression with a next day provisional delivery. Based on our clinical experience and supported by numerous scientific and clinical trials worldwide, this treatment concept performs superiorly to the conventional approaches with massive grafting procedures and a two stage surgery.

or complete coverage of the site is of great importance.

The soft and/or hard tissue enhancement in the majority if not all of the cases in the esthetic zone is absolutely necessary in order to achieve a highly esthetic and natural appearance of the implant crown. This presentation will focus on the methodology of the implant site development, especially in demanding esthetic cases, on today’s knowledge of the biology of different materials and prosthetic type selection and give some guidelines to achieve optimum esthetic results. Finally, new approaches with the help of clinical case presentations will be discussed.

How good are xenogenic collagenous matrices for soft tissue reconstruction?

One of the goals in periodontal plastic surgery is to completely cover the recession defects and change the phenotype of the soft tissues. Usually we use autogenous soft tissue grafts being either the connective tissue graft or the free gingival graft.

Both of these grafts require harvesting from the palate  which makes it sometimes traumatic and more painful for the patients. To reduce patient morbidity over the last decade xenogranous collagen matrices have been introduced as replacement grafts to autogenous ones. However these materials have limitations and there is also a learning curve when utilizing them. This lecture will cover various collagenous matrices for soft tissue reconstruction and will show clinical results when using these materials and comparisons to autogenous grafts. Furthermore, their limitations and proper indications will be shown in order to understand what is the right approach when using these materials.

Current concepts in reconstructive periodontal surgery

The management of teeth periodontally compromised by deep intraosseous lesions has been a major therapeutic challenge for many years. For this purpose, various surgical techniques and biomaterials have been investigated in an attempt to reconstruct lost or damaged periodontal tissues.

Thus, reconstruction of periodontal tissues may be achieved by application of barrier membranes, grafts, biological agents, and their combinations. However, although reconstructive procedures are widely accepted as safe and effective, there is a high variability in treatment outcomes. Recently, minimally invasive surgical and non-surgical treatment options have been introduced with the aim to minimize tissue trauma, decrease patient morbidity and simplify the treatment procedure.

The goal of this lecture is to update clinicians on the recent advances in biomaterials and surgical techniques in reconstructive periodontal procedures.

New aspects of soft tissue management for bone augmentation

In the new digital world very little has been changed for the manipulation of the soft tissues. The planning of implant positioning, the amount of bone grafting and the delivery of the prosthetic work can be done digitally.

However, the manipulation of the soft tissue for a bone or tissue augmentation have not being influenced. Still planning for guided bone regeneration and the amount of necessary soft tissue manipulation can be visualized digitally.

The lecture will focus on the analog execution of incision flap, design and suture in the aesthetic zone and the non-aesthetic zone, from concept to execution of new developed surgical techniques for achieving predictable result. In these cases, soft tissue assessment before surgery is key for success.

Immediate implant placement: from a single tooth replacement to a full-arch rehabilitation

Today’s Dental World revolves around implants; teeth are preserved, edentulous patients desire fixed solutions.  In such an environment GBR became a standard procedure in our dental offices. On the other hand, our patients’ most frequent wishes and requests is effectiveness.

Is it possible to harmoniously combine the treatment of bone loss, the need for GBR and be effective with that?

Tooth extraction is an introduction to the world of dental implants for the patients. The most effective way to replace a tooth after an extraction is immediate implant placement. Unfortunately this well-known approach is still controversial; in literature this concept is divided between authors that declare it predictable and those which claim it to be an unpredictable technique. Looking from the patient’s and clinician’s side, this approach has many advantages such as saving time, being effective and bringing some biological advantages.

In this lecture, the biological principles of bone loss after tooth extraction will be discussed and presented as well as their consequences on our clinical approach. Where the limits of immediate implant placement are and what are the prerequisites for a safe and predictable implant placement in such an environment? Do we need bone substitutes, can we restore the tooth or an entire arch on implants placed in an extraction socket?  Through clinical cases, techniques and procedures such as how to get from a single tooth replacement to a full arch rehabilitation and how to get from anterior zone to posterior zone solutions, will be presented and discussed step by step.

Digitally driven implantology

Osseointegrated implants are a practical alternative to traditional prosthetics; however, the challenge is to make a prosthetic replacement on the implant from a functional and aesthetic point of view.

High precision in planning and performing surgical procedures is important to ensure success rates without causing iatrogenic damage. This can be achieved by computed tomography (CT), three-dimensional implant planning software, 3D-guided template-making techniques, and computer-guided surgery. The success of implant therapy depends primarily on proper treatment planning and properly performed implant placement surgery. Improper implant placement is a very common occurrence that often complicates clinical and laboratory procedures designed to make prosthetic rehabilitation. This actually requires the close collaboration of prosthetists and surgeons to work together as a team to facilitate the precise design of the surgical guide. The lecture will show the development of a digital plan and the course of implant-prosthetic rehabilitation with surgical guides

Modern graftless solutions - The use of ``zirconia solutions`` in implant dentistry

Contemporary implantology is constantly evolving thanks to the evolution of materials, technology and diagnostic methods, following a very high variability of materials and solutions.

The digital workflow has improved the fluidity and quality of the dentist-dental technician dialogue. This is a special synergy and a very key of a long-term success.
The use of “zirconia solutions” allows to treat complex cases through a completely digital flow thanks to the CAD/CAM procedures. Furthermore, the new monolithic solutions allow to maintain functional parameters, a good longevity and an excellent biomechanical stability.
The aim of this lecture is to provide the clinician and dental technician with an innovative working method, based on 3 crucial elements: precision, communication, quality. There will be a theoretical part and an interactive pratical part, in which to discuss all the prosthetic solutions. Screwed, cemented, conometric and mixed solutions will be analyzed, with the use of zirconia and its new uses.
